Equinus is significantly underdiagnosed. Many patients with persistent plantar fasciitis, recurring arch pain, metatarsalgia, or even chronic low back pain have never been evaluated for equinus — even after multiple rounds of treatment. A brief physical exam can identify it in seconds.

What Is Equinus?

Equinus is a limitation in the upward range of motion of the foot at the ankle joint. In a normal gait cycle, the foot must dorsiflex (bend upward) at least 10 degrees past a 90-degree angle with the leg. When this motion is restricted — by tight muscles, tendons, or a bony blockage — the condition is called equinus.

The term comes from the Latin word for horse, referring to the toe-down position characteristic of a horse's foot. In the extreme form seen in untreated club foot or neurological conditions, the heel never makes ground contact. But in the far more common and clinically significant mild-to-moderate equinus seen in everyday patients, the restriction is subtle — yet its effects throughout the entire kinetic chain are profound.

Causes of Equinus

Equinus has three primary categories of cause:

Muscular and Tendinous Equinus (Most Common)

Tightness in the calf muscles — the gastrocnemius, soleus, or both — and/or the Achilles tendon is by far the most frequent cause. This type is known as spastic or flexible equinus because the restriction is in soft tissue rather than bone, and motion can be passively increased with proper stretching.

Contributing factors include:

  • Years of wearing high-heeled shoes, which shorten the Achilles over time
  • Prolonged cast or boot immobilization, which causes adaptive shortening
  • Athletic training without adequate calf stretching
  • Normal aging changes in tendon flexibility
  • Obesity, which increases strain on the Achilles and calf complex

Neurological Equinus

Spasticity from neurological conditions such as cerebral palsy, stroke, multiple sclerosis, or peripheral neuropathy can cause persistent equinus by creating abnormal muscle tone that prevents relaxation of the calf complex.

Osseous (Bony) Equinus

In some cases, a bony prominence at the front of the ankle joint physically blocks the foot from dorsiflexing normally. This is called an anterior ankle impingement or bone spur and may require surgical treatment if symptomatic.

How Equinus Causes Other Foot Problems

When the ankle cannot dorsiflex properly during walking, the body compensates. These compensations occur every step, thousands of times per day, and they redistribute forces in ways that cause pain and deformity throughout the foot and lower body.

Plantar Fasciitis and Heel Pain

With restricted ankle dorsiflexion, the windlass mechanism of the foot is disrupted. Extra strain is placed on the plantar fascia with every step, leading to microtears and the classic heel pain of plantar fasciitis. Patients who treat plantar fasciitis without addressing the underlying equinus frequently experience recurrence.

Metatarsalgia (Ball-of-Foot Pain)

Unable to rock over the heel and ankle normally, the body compensates by transferring weight prematurely to the ball of the foot. This overloads the metatarsal heads and causes pain, calluses, and stress fractures in the forefoot.

Bunions and Hammertoes

Compensation patterns from equinus — including toe-out walking and increased pronation — generate lateral forces across the forefoot that progressively deform the metatarsophalangeal joints. Bunions and hammertoe deformities develop more rapidly in patients with untreated equinus.

Achilles Tendonitis

Chronic tightness in the Achilles tendon creates repetitive microstress at the tendon insertion and midsubstance, predisposing it to tendinopathy and, in severe cases, rupture.

Knee, Hip, and Lower Back Pain

Compensation for equinus often begins at the subtalar joint (excessive pronation, rolling in of the arch), then travels upward. Increased internal tibial rotation places abnormal stress on the knee. Altered pelvic tilt from the gait compensation can cause low back pain. Patients sometimes see orthopedic surgeons and physical therapists for knee or back complaints without the ankle range-of-motion restriction ever being evaluated.

Diagnosis

Diagnosing equinus is straightforward. During a physical exam, your podiatrist will measure the range of motion of your ankle using a technique called the Silfverskiöld test:

  • With the knee extended, ankle dorsiflexion is measured. If less than 10 degrees past 90° is achievable, equinus is present.
  • The test is then repeated with the knee bent to 90°. If dorsiflexion improves significantly with the knee bent, the gastrocnemius muscle (which crosses the knee) is the primary source of restriction — a gastrocnemius equinus.
  • If restriction persists with the knee bent, the soleus or Achilles tendon is involved — an equinus of the soleus-Achilles complex.

This distinction is important because it determines the type of stretching protocol — and, if surgery is needed, which structure requires lengthening.

Imaging (weight-bearing X-rays) may be used to assess the foot structure, rule out osseous equinus, and evaluate any secondary deformities.

Treatment Options

Stretching Programs

For flexible muscular equinus, a consistent stretching program targeting the gastrocnemius and soleus is the foundation of treatment. A physical therapist can supervise and progress the stretching protocol. Key exercises include:

  • Straight-knee (gastrocnemius) calf stretch — held 30 seconds, repeated 3–5 times, 2–3 times daily
  • Bent-knee (soleus) calf stretch
  • Stair heel drops (eccentric loading)

Results from stretching require consistency over weeks to months. Intermittent stretching will not provide lasting gains in tendon length.

Night Splints

Night splints hold the foot in a dorsiflexed position during sleep, providing a sustained low-load stretch over the calf complex for 6–8 hours. They are particularly effective for gastrocnemius equinus and are frequently combined with daytime stretching for plantar fasciitis management.

Orthotics and Footwear Modifications

Custom foot orthotics can help manage the secondary deformities caused by equinus — controlling pronation, redistributing forefoot pressure, and supporting the arch — while the underlying tightness is being addressed. A heel lift incorporated into an orthotic or shoe can temporarily reduce strain on a tight Achilles tendon.

Bracing and Physical Therapy

Ankle-foot orthoses (AFOs) or dynamic splinting devices can provide sustained dorsiflexion stretch for patients with moderate-to-severe equinus. Physical therapy modalities including ultrasound, deep tissue massage, and instrument-assisted soft tissue mobilization (IASTM) may accelerate soft tissue remodeling.

Gastrocnemius Recession and Achilles Lengthening Surgery

When conservative treatment does not produce adequate improvement in ankle dorsiflexion — and equinus continues to drive pain or deformity — surgical lengthening may be recommended. The two main surgical options are:

  • Gastrocnemius recession — the gastrocnemius fascia or muscle is lengthened just below the knee. This procedure specifically addresses isolated gastrocnemius equinus, preserving soleus function and minimizing the risk of over-correction. Recovery is typically 4–6 weeks with protected weight-bearing, followed by physical therapy.
  • Achilles tendon lengthening (TAL) — the Achilles tendon itself is lengthened. This is reserved for more severe or combined equinus. It requires careful technique to avoid over-lengthening, which can cause heel walking (calcaneal gait) and weakness.

Surgery for equinus produces significant, durable improvements in foot function and is often curative for the downstream foot problems — plantar fasciitis, metatarsalgia — that resulted from the restriction.
